Maybe I missed something, but I find it hard to understand why Continental should be fined when they really had no way to get the pax off the plane.
Mesaba did eventually offer to deplane the pax (although it was very tardy) @ 0430. XJET dispatch declined to deplane at this point.

CO has staff at Madison and Des Moines, it was not an emergency, why RST?
