The SQ agent at the ticketing office dissuaded me from ordering the steak as she said that all meats must be cooked as 'well done' due to airline regulations. Meals are delivered to aircraft frozen (but already cooked) and have to be heated up in the ovens.
The only airline I know who cared how the meat was done was on JL in F class. But that was a long time ago before 'Mad Cow' became a household name.
