There is no fee involved if you are using SkyMiles. I have had a DL phone agent book a double open jaw itinerary for me NW FLL-DTW-LHR out and KL BCN-AMS-JFK back. You do not need to go to nwa.com except to try to see if there is something there that you would like to ask them to find for you.
