I've never seen these - on American. Western Air Lines used to give out "Flub Stubs" good for a free drink onboard when any employee messed up in any way. It was a good way to provide an instant acknowledgment and apology, and was probably cost effective - particularly given some pax kept the Flub Sutb as evidence of WA's concern and as a petit trophy.
