I have had similar problems, although not quite as bad as described here.
My problems with the EEO certificates begin at reservation time; the agents don't know how to book them correctly. I check the reservation on the internet and see the errors, which I then attempt to correct. Usually, by the time I get to the hotel, the record has so many comments that no one can screw it up!
I also check my bills during stays so that at least some errors are apparent before checkout. Fixing these problems when you're rushing to get somewhere is too difficult. I have never had a "free" night billed 2 months later, but I would hit the roof if that ever happened to me. I don't have any records that much later.
