I'm pretty sure the change from Preferred/Gold to Silver/Gold/Platinum coincided with the start of the Continental alliance. It looks like the deal was made in 1998, so 1999 was probably the first program year on the 3 tier system. I can still remember having to use 500 mile upgrade certificates my first couple years as Preferred before the launch of EUA, in the 1996-98 timeframe.
