To add my data point:
* Bought my 7-day card late in the day on 9/3 for $27
* Card died the morning of 9/5. Got a damaged card form from a booth and mailed in that afternoon with my receipt from Hudson News. It has to be postmarked within a few days and that was the Labor Day weekend, but I added a note about that on the form.
* Called the first week or two in October to ask and they said they were backlogged and still processing requests from August.
* On 10/24 received a check for $23.16. So while I had technically used it on 2 days and had just started my 3rd, they gave me a refund for 6 days.

I am happy with the outcome. Although for future trips, I might stick more with 1-day passes and pay per ride as it was extremely frustrating when it all happened. I wish they would just go RFID throughout the system, although I still need to mail in my flaky SF TransLink card.
