Agree it may be an issue of terminology with asking to "combine" eliciting a response of "the fee is..." but I have in the past drawn miles from two different accounts to cover mileage upgrades for two pax when one account did not have sufficient miles for the award. I'd try the old hang up and call back if an agent says no.
