As far as fasttrack - during my last stay in Bkk, I used my local office's TA for a variety of side trips in SE Asia. They offered a service (free of charge for those who purchased tickets from them) whereby someone met you to check you in,then fasttrack you through immigration, and on the return flight back into Bkk, meet you near your gate and again fasttrack you. And yes, immigration lines at certain hours (eg early morning for all the USA return flights) are super super long....
