Can you please clarify the way the medical transportation works?

Does it apply to me and my immediate family?

Does the trip (airline, hotel etc) have to be charged on the specific Amex Platinum card or does the benefit apply no matter how the trip was paid for?

Just planning a trip to Peru later this year and contemplating trip insurance and medical insurance.
