I'm not sure if Makaha has it, but it it's properly spelled Mākaha, then it's the first, MA' KAHA, much like Mānoa is properly pronounced MA' NOA with a hard emphasis on the MA. If it's just plainly Makaha, then there's no hard emphasis. At least that's my understanding of it.
