3 pages!!?? I always thought it was 2. But a quick check shows that the IATA rules (confirmed by the Delta web site) say 1!

Passports and passport replacing documents of all non-South
African nationals must contain at least 1 unused visa page
for entry/departure endorsements by South African
Immigration Service. Please note that this does not include
the page reserved for Observations or Amendments and
Endorsements".
Which seems a bit more reasonable.

Dreadful experience but be thankful she was let in at all. I attended a local conference where the bigwig scheduled to talk was turned back at CPT because his passport lacked the requisite page(s). The sponsors were livid, of course, and humiliated because their star turn didn't show.

I had a similar experience with Tanzania where they refused to issue my visa because of lack of (I forget how many) pages. I had to apply for an emergency temporary passport the issuing of which, amazingly, our Dept. of Home Affairs actually managed very well. ^ But which my travel agent lost after collecting it from the embassy. That was the day before I was due to leave. The next day they found it. ^ (It had fallen behind a desk. ) Perhaps the most bizarre part of this whole saga was that I stayed strangely calm throughout - not my usual reaction!
