$450/yr. You can only use the lounge you're ticketed on - not just any affililate. (e.g. You see a NWA lounge but you're ticketed on Cont, you can't use the NWA lounge). 2/1 1st class tix - you have to use their travel agent and the tickets are 2x what you'd pay using the airline direct. I tried, unsuccessfully, many times to use them. It was always more the same ticket direct from the airline. Concierge is a joke. Maybe good for certain event tickets - don't know.

Overall the benefits seem elusive.
