Whether or not a hard line is appropriate would depend on the nature of the failure which allowed the mistake to occur. If it was intentional or negligence then come down hard but come down hard on the airline, not the traveling public. If you come down hard on honest mistakes and oversights airlines will cover up their mistakes instead of self-disclosing as Southwest did in this case and the case with the skin inspections.
Wait a sec, Southwest self-disclosed the issue in this case? Both WSJ articles on this said that an FAA inspector was the one that discovered the bad parts.
