IME, the credit card does not need to match the person's account.

OP - first, sorry for your father's situation. I think option 1 & 2 are the best, and as a family member with the same last name, it shouldn't be any trouble booking awards over the phone for yourself. But, I don't think you'd be able to call to redeem his miles for your girlfriend, since he'd have to sign a bluey (I think). Online redemptions (read: all-UA itins) shouldn't be a problem though.
