While a notation in the reservation is worthwhile, it is only a request for connecting rooms and by no means a guarentee at check-in. I have found a call directly to the property and speaking with the HHonors manager to be the most effective means of obtaining connecting rooms. I explain that I am HHonors Gold and have a request for connecting rooms. I just recently did this for a stay at the SFO Doubletree last week. I placed the call 2 weeks in advance of our stay so the manager would have time to investigate the possibilities and block out our rooms in advance.
