Name has to match name on your file, which of course has your FF number.
As an example, back when you were allowed to use only your middle name on your ticket (TSA not CO) I ran into this when I took an international flight that was booked by my former companie's travel department. They put in my full name and number, but I didn't get credit for the flight (I called and got it straighted out, but at least it was my name).
