Well the strangest thing just happened. 5 minutes after I posted this, I got an email from the VP of AAdvantage Customer Service. It seems canned. It states that award tickets were issued today (despite our travel being over now) for 37,500 miles to my fiance's name (using my last name though even though we're not married).

I logged into to AA.com, and the entire 37,500 mile reinstate is completely gone making it like nothing ever happened. There are no reverse line items or a re-decuction of 37,500 miles. All activity related to these tickets has just vanished. It's sort of where we were before I called on 08-20-2009, with the only caveat now that I paid the $30 ticketing fee at the airport.

It seems I still need to pay/cancel the $150 to get my original 25k back from the Vegas ticket.

Either AA is trolling this forum and immediately read and started rectifying this, or that is the strangest coincident. At this point, I'd be happy if it was just fixed.
