1. 09-14-2008 - Reserved award travel for my fiance to meet me on a business trip in Vegas in January 2009. 25k mile award.

2. Late December, 2008 - Canceled reward ticket because I broke my foot and having her in Vegas with me on crutches wasn't our original plan. AAgent told me I could keep the ticket up until 09-14-2009 and use it for any AUS-LAS ticket without charge. Since I have family in LAS we decided we'd hang onto it. I was informed to keep the ticket number (not locator) to re-use this ticket or refund it before 09-14-2009.

3. July 2009 - I booked an award ticked for my fiance for AUS-YYZ leaving on 08-24-2009 in the early AM. It was a 37.5k award ticket.

4. 08-22-2009 - I called to cancel the 09-14-2008 award since we will be unable to use it before hand. I gave the AAgent the ticket number for the AUS-LAS award ticket. She verified its expiration of 09-14-2009. I was told I'd receive my 25k miles back for a $150 charge. I gave my CC# and all was good.

5. 08-24-2009 - At 1 hour before flight time, I receive a call from my fiance. She said the gate AAgent informed her her ticket had been canceled. There was nothing the gate AAgent could do.

I immediately called the PLT desk and ask for assistance in the most expedited fashion. The gentlemen AAgent I spoke with looked into it and after 25 minutes had issued a new award ticket. At the same time, I logged into my AA.com account to see that I had just been reinstated the 07-30-2009 37.5k award (indicating which ticket was canceled). The AAgent informed me he'd have to use a 50k award and my fiance would have to pay a $100 expedite fee plus a $30 airport service fee plus the taxes for the flight (~$50) at the airport.

He said, I should email Customer Relations to get everything sorted out later.

Finally, my fiance waited in queue again and made it on the flight with just a couple minutes to spare. There were no other issues with her travel. I highly praise the gentlemen I spoke to who got an entire ticket issued within ~25 minutes.

Today - I just arrived back home and looked over everything, and now there seems to be many issues with my Credit Card and my Award statement. I've already written my email to Customer Relations and have it saved in my drafts folder, but now I'm not sure what's going on.

Facts:
08-20-2009 - 37.5K was redeposited into my account
08-21-2009 - $50.xx was refunded to my Amex (the taxes from the wrongly canceled YYZ flight).
08-22-2009 - A $50.xx tax charge and $30 ticketing fee were charged to my Amex
08-24-2009 - My fiance's return tickets were both 'Y' class (not sure if this is normal for an award ticket). When we went standby (on separate PNRs), she was put ahead of all non-elites (after linking to me, she got ahead of Gold).

Non-linear things (as of Tuesday, 08-25-2009):
1. I've not been charged the $150 AAward refund fee from 08-20-2009. I'd have expected this charge to occur when the refund for the taxes occurred.
2. I've not been charged the 50k miles for my fiance's trip. If I recall correctly, AAward mileage deductions occur nearly immediately.
3. My fiance was never charged the $100 late booking fee at the airport, she was charged the taxes and $30 airport ticketing fee.

As it stands now, if nothing changes, it seems I've gotten a better deal. I simply paid an additional $30, got 37.5k miles back instead of 25k and didn't get charged the $150 cancel fee.

I suppose this could all sort out over the course of the next few days, but my experience has been AA charges post to my Amex much quicker than 4 business days (6 days), and reward miles get deducted very quickly.

What do you think is going on?
AA does have some sort of audit procedure in place (for example FTs posting about getting miles for an award travel where there was change in flights and the GA incorrectly coded but later the mile were taken back).

My suggestion is to hold tight. It would hard to speculate exactly what the TA did although you may get a number of FTs telling your their opinion/version. At worst, AA will go back and reverse miles and send through charges on your credit card (so don't think your chickens have been counted). At best nothing happens and you are to the good.
