Actually, they do make exceptions -- at least in theory. In practice, it is rather ridiculous.
I was thinking of making a stop in MXP on my way back to Tel Aviv. I called Delta and asked how much they would charge if I made this change. The answer: $2900!
I can fly TLV-MXP r/t for $600 (in Y) even at the most expensive time of the year.
