I've found it to be pretty easy. You have to call the mileage plan desk, but there is always plenty of availability.
I usually just call the MVP desk and they can do it from there, even for other family members
As far as availability goes.. yes - not bad... but there are limited 'saver seats' but its only another 5K miles to the top level... well worth it for some flights
