I think the concern is that while pilots and executives of airlines all want to run a safe show, the aviation business is a learning experience. Many accidents bring new lessons, and revised procedures as a result.

I would say there are few incidents of what one might term "willful negligence" which could perhaps be asserted with the D10 rear cargo lock.

However, I don't believe AK executives wanted to put their planes at risk, yet they ended up doing so, and there was an accident. Same with the revised procedures AA was using that led to the ORD D10 crash. I am sure there are some executives who will never be able to sleep soundly again because of these decisions.

With all the hogwash the Unions throw out it really muddies the water, which makes making sure corners that shouldn't be cut aren't much harder. I think there is no question "fuel management" is a relatively recent science as airlines identified how expensive it is to carry around unused fuel. And sure technology advances do help in better planning. However, I am not sure we can just give away free passes when it comes to safety under the belief that everyone will do the right thing. All it takes is one intimidated pilot who has other issues to make the wrong decision. I would want to make sure the pilot is never put in that position, but again, their own Unions make sorting truth from fiction that much more difficult IMHO.
