So what name should I put?

Which name does she fill out on her everyday forms? What's on her credit card and driver's license? That's the one she should use. If ever there are any questions she can show those other pieces and it will be clear it's her. it is also good if the name on the ticket and on the passport match.

I suppose that this is just a way of saying one is her maiden name and one is her married name. If she usually goes by the married name, which is likely, that's what she should use.
