In all seriousness, recently I have left out my title and middle initial because it looks sloppy to me on the BP. With the new TSA rules, though, I am going to get the small print on my next trip!
I've found that any award flights you book over the phone always include the title. I haven't used miles for a flight that I can book on United.com in years, so I'm not sure if you can remove the title for those.
As a side note, I have my wife as a saved passenger in my profile and when I add her to a reservation, it has her first name and middle initial in the box for her first name. Annoying, but easily edited.
