My understanding is that a note is made in the comments section of PNRs referencing the other PNR. I think an agent could see it only if they were looking for it but that it is not something that autorebooking tool would look for if your outbound leg is late, for example.
I have mostly used it to link a PNR when traveling with someone on a different PNR.
