There really isn't an industry standard for premium economy. The "economy seat with more room" is your own definition. For example, Air France sell intra-Europe premium economy without the extra room. Other airlines feature previous-generation business-class seats in premium economy, so that also isn't "an economy seat with more room".

Not necessarily. Usually full-fare business class will cost about 1.5-2x the price of full-fare Economy. What you're describing is a situation where cheap, restricted Y fares exist while cheap, restricted C fares don't. Which, to be fair, does happen often, but not nearly always
