95% of my travel is paid for by my employer (75k miles this year, which is light for me). I make a good salary, but wouldn't really consider paying the sorts of money I see charged for upgraded cabins. If I fly premium, its usually an upgrade instrument earned by other flights. There are the rare occasions (tonight, for example), where a discounted first ticket sneaks its way past our travel agency.
