I like the Pinnacle crew that was dead-heading an empty plane somewhere and decided to exceed the operational ceiling to test out the A/C. IIRC, both engines flamed out and they crashed shortly after. Link

EDIT - can't forget last week's CRJ event in ATL when one of the main gear didn't come down (IIRC). No fatalities, but hardly crew error. Still, they do have a good safety record.
