Are you ordering through CO or travel agent? A lot of special meals have been discontinued so although the travel agent enters it correctly CO would not order if not provided anymore.
Also could be down to the agent entering wrong code. Some agents put it in the OSI field and not the required SSR field of your reservation.
Or the agents are just missing it, make it known so something can be done to rectify.
