Personally, I think that we're all being distracted by the sexism issue, and this would have been a very different conversation had that not been raised.
Yes, not giving Doreen her menu when they were handed out to everyone else was technically a service lapse. In the big picture of what might go wrong on a flight, I see it as verrry minor. Things like that have happened to me in F from time to time. Had she not walked back to the galley, the FAs would have noticed that she had no menu, and it probably would have been sooner than later. In my experience, cabin crew are always walking through the cabin (at least until pax start to go to sleep), someone would have noticed, and she would have been given the menu along with a sincere apology.
Let's keep in mind what Henky said. The FAs are not servants. Nor are they perfect, any more than I am. Sometimes people make mistakes. For heaven's sake, though, keep it in perspective. Doreen wasn't deprived of anything. Her safety wasn't jeopardized. I'm confident that no one here believes that Doreen would have gone the entire flight with no one noticing. In fact, the FAs would have noticed that she didn't have a menu when they arrived at her seat to take her food preference. That is the latest time at which this would have been remedied.
When I fly, I don't expect perfection. Neither should anyone else. Mistakes happen. In fact, the mark of true professionals is how they handle those few situations in which mistakes are made. And on that scale, IMO, BA crews score very high.
