Having lived in Milan, I will give you a step by step.

Follow the above options in terms of getting into Milan. Upon arrival in Centrale, proceed to the Green line subway (located directly in front and underground of the station).

From there, buy a 1 euro ticket and take a southbound train to Cadorna (ride takes 13 minutes). At Cadorna, take the MXP express (very easy to find, good signage in English), which is up the stairs, straight ahead. The ticket is about 11 euro.

The trains run every 30 minutes, at :27 and :57. Ride takes 40 minutes.
