ok, that is just a micro-sd to sd adapter. You slide the micro-sd card inside the adapter and then if you have an sd card reader or an sd card slot on your computer, you can read/write the micro-sd card using that adapter.
The theory is that many people have some way of reading/writing an sd card on their computer, but no way of reading/writing a micro-sd card. That would be true for me. So to work around that, many micro-sd cards just come with those adapters now. If you don't need it, don't worry about it.
They work fine, just like any other memory card adapter. The adapter just converts from one form factor to another one.

Also, if one is in a pinch, they can use your adapter to make the memory card large enough to fit in a digital camera that accepts SD cards.
