Didn't have that issue myself going CVG-LGW earlier this month we were slated to get in an hour early ([td] on an overnight flight).

When they were taking orders, I ordered the steak and asked for everything at once so I could sleep. No problem, had everything (Steak & Salad on one tray) when everyone else was getting their dinner roll....

I believe the problem may have been you not mentioning at order or anytime before 2:15 into flight. Hate to pin it on you but I don't see it being anyone else's fault.
