I gathered from the above that a last name change as a result of a life event (marriage/divorce) did not require documentation, but that various other changes did.

Interesting because when I changed my name about 7 years ago when I got married, I had to do it in writing via mail and had to include a copy of my marriage license. It looks like they are a little more relaxed about those types of routine name changes now.
