Yes, phone only booking.

And yes, I believe Fifth Night Free works--I think you get the whole fifth night free. Worst case, you still pay the extra 1K points for the fifth night, but get the base points free.

EDITED TO ADD:
Just to be clear, not all hotels offer upgraded award rooms, though many do. They also can subject them to capacity controls at the hotels discretion. Depending on hotel/category, the upgraded rooms can be between 1,000-2,750 extra points per night. Some hotels have Junior Suites available, some have Club Rooms, some have view upgrades, etc. It varies a lot, but it's always worth calling to see what is on offer at that property.
