When I was setting such an account for my wife and me (it's called Mutual Fund account), I've faxed my request to HHonors, along with a copy of the marriage license. It took them just a couple days back then to set it up. I've noticed though that lately it often takes much, much longer for their CS to get to any request members may be mailing/faxing. I'd say that 4 weeks is enough waiting time, and you may be better off resending your request.
