I often add flights, etc... that require agent assistance. I have the agent make the reservation and ask them to place it on hold. I then go online and book it myself - and thus do not have to pay the booking fee.
Except revenue tickets with an airline segment other than AA cannot be put on hold. However, more often than not, you can buy those online.
