You will have to go through customs. Even if you were connecting in the same terminal, all connecting passengers must enter the USA. Not as easy as the EU...
b) The shortest connection time I have is 3 hours (number 1). Is that enough? The other two give me about 4 hours each time.
2 hours is enough, so you will be fine.
