I'm not sure about experience of others, but to me I think all these duplicated data entry happens all the time...

when I tried to book for an upgrade, they would ask you at that time already the ticket number. usually I didn't have it (yet), but in cases where I did, I provided it. but then when I called to issue the award, they would ask for it again. only in cases where I insisted that "you have got it already, pls check", that they would go to check the record and found it.

when I booked an award ticket to China, they claimed that they need to include the travel document details in the booking. then when I did online check-in, it would ask for it again. even if you provided everything correctly during online check-in, if you notice carefully you would see that the check-in agents always re-inputted everything again when you checked in at the AEL station / airport.

that's why now I always try not to provide any of these details any more (e.g. when just booking an upgrade and not issuing yet) even if I have it - would wait for the last minute so that I don't have to waste time providing these again and again...
