I would agree with the assertion that US is not generous in terms of its long-haul upgrades, but no other airline that I'm aware of allows a complete free-for-all on systemwide upgrades, regardless of your elite level.
AA is restrcited only to exclude "O" everywhere. "Q" is also restricted Internationally. (Ignoring "I" fares as US has not int'l F).

Until recently the lowest fares were rarely non-upgradable. They've made the dirt cheap ones O's and Q's now, but you can still generally get where you want to go and be upgradable for <750 IME.

Oh, and they're also confirmable at booking if inventory is available.
