First off, to the OP, I'm sorry to hear about your situation. My grandmother also had this terrible disease, so I can certainly sympathize with your situation. I hope DL will be able to do something for you, it would be great if they would refund the ticket due to medical reasons. I'm not sure if they will, but it is certainly worth checking out. I assume they would need a letter from a doctor.

Please note that this is a terrible suggestion, mainly because this would be fraud, plain and simple. I can't beleive the amount of people who suggest (in general) disputing a CC charge because they don't like something the vendor has done.

It is unfortunate if Delta doesn't offer a refund based on a medical letter, however, they are probably not required to. However, if you read your CC agreement, you'll note it says something like you can dispute charges if the CC holder did not authorize the charge in the first place, or if the vendor didn't provide the service per the terms of the transaction. Based on the OP's post, this is not even close to the case.

On the other hand, if the card that paid for the ticket has travel insurance as a benefit, than the OP will likely be able to get a full or partial refund absed on this.

Having no affiliation with Delta, I am happy to explain the fee to you. The fee was raised to this amount to remain competitive with otehr carriers. Others do it, so DL does. Also, they do it because they can.
