I searched the forum, but found nothing relevant and the UA refund line sucks.

Does anyone have the mailing address for where to return a paper mileage plus ticket? I have a paper ticket from early last year that was canceled, however I still need to mail back the old paper ticket to get the 60k miles back. Does anyone have the mailing address where paper mileage tickets have to be sent back to?
