Once you've started the trip, it's often almost impossible to get any refund on the ticket. I think it's because a one way ticket is usually very expensive, and would take up the value of a discounted RT.

As a Plat, I certainly wouldn't have blown miles to upgrade a domestic itinerary. I'm usually 95-100% on domestic upgrades, but perhaps it's harder out of the cities you're flying out of.
