Thanks to LAX21 for explaining how you can get an E mail "receipt" - showing what you have paid - online in this thread:

So both problems seem to be solved: for unpaid refunds, threaten the "Hairbrush" CC letter to refunds and/or credit at flybmi dotcom to elicit a speedy result; for award-booking receipts go to MMB (cos it doesn't appear in My Itins) and select E-Mail at the top, you can even input a new E Mail address.
