AC has this policy where

1. Fare paying passengers get first meal choice
2. Upgrade certificate gets second meal choice
3. Last minute upgrade certificate gets third meal choice
4. Courtesty upgrade passengers get last meal choice (i believe its sub-categorized by status)

I dont know where Aeroplan Reward passengers would fit. However, in my experience crew told me meal priority is printed on the boarding pass (ML*1) and manifest (list of passengers give from ground staff to crew to arrival ground staff again). Whether crew follows this policy is erratic because they sometimes serve me with the others, and sometimes they don't.

NRT has a departure tax paid @ the airport on top of the regular airport tax. Used to be 500 yen (but I'm talking 10 years ago). I believe YVR has an airport improvement fee of $5-10 not included in airport taxes too.... similar concept.
