It may be because I have a 2 year-old US account with literally 500 miles in it (I have some US flights scheduled for 2004, but earning is going to UA, not US). If I can any other offers or similar informationa and a usable link (the one provided has no info) then, I'll post here.

TERMS & CONDITIONS: Offer valid only to new cardholders approved
between 1/20/04 and 6/30/04. Triple miles will be earned on all flights taken
after card approval date, purchase of tickets, and completed travel by
6/30/04. Triple flight mile bonus valid toward Preferred status on
all mileage qualifying fares for travel on US Airways, US Airways Shuttle
or US Airways Express operated flights between 1/20/04 -6/30/04 and
purchased with a new Dividend Miles Visa Credit Card approved on or after
1/20/04. You will automatically be registered for the Triple Miles
offer once you are approved for the Dividend Miles Visa Credit Card,
activate your new card, and purchase your US Airways tickets using the new
card. This offer is not valid with the Dividend Miles Visa Check Card
or Dividend Miles Visa Business Credit Card. Offer not combinable with
any other flight bonus mile or Preferred status offers. This offer
excludes travel on other airline partners such as United, Lufthansa,
etc. or on codeshare flights operated by other airlines. All Dividend
Miles terms and conditions apply.

If account is opened on or before 6/30/04, Platinum cardholders earn
10,000 bonus miles, Gold cardholders earn 7,000 bonus miles and Classic
cardholders earn 5,000 bonus miles after you make your first purchase.
Regular new account bonus miles: Platinum cardholders earn 7,000 bonus
miles, Gold cardholders earn 4,000 bonus miles and Classic cardholders
earn 3,000 bonus miles after you make your first purchase.
