Only if you are connecting in Europe for a flight from/to Zones 1 and 2; connecting in Europe for long haul does not incur extra miles.

Yes, but you would only be allowed one stopover of more than 24 hours.

So you could for example fly BHX-FRA-(no extra charge for connection)-JFK-IAH-LAX but you could not have long stopovers in both JFK and IAH, only one or the other.
