Why not use the electronic feature on the Delta website to request missing mileage? Just last week I sent the details of a missing NW segment from March 3 to Delta (for SM account) and within about 4 days I got an email response from Delta saying the miles have been credited.^ There's even a text box to enter the "gory details" as needed...
I just did the same re a missing NW segment from February 13. I received an e-mail response and the miles were credited within 24 hours. No boarding pass or other documentation was requested or required.
