I did 5 transatlantic segments in July/August on A330s, on none of them was the forward lav off limits. Which is a good thing too, as I was in row 1 I would have been annoyed to have to go to the back of Envoy. Probably either the captain as was said or who knows, maybe a problem with the cockpit door lock or something. On UA I have seen a few captains declare the forward lav off limits. Saw it a couple times right after Sept 11. Not as common now.
