Me too. It's so rare though.
Maybe they only clean them when there're complaints, or they themselves want to go to the bathroom.
The only toilet in the 777 that is in better shape is the most forward one in J. The pilots use it. Once in a while I even see FAs preventing people from visiting that toilet.
Don't know if there's a policy of not allowing pax to use it.
